•  Daftar Riwayat Hidup (DRH) dengan mencantumkan alamat lengkap;
•  Surat Pernyataan bersedia ditempatkan di seluruh Indonesia dengan dibubuhi materai Rp.6.000,-

b. Pendaftaran :

4. Seleksi Administrasi :
